PAGEANT FORM SECTION:

Complete the forms below online, print them out and mail them to the pageant office at the address below. The form deadline is the same date of your last payment. You should take care of your fees and paperwork before you begin to work on your wardrobe and fitness.

VIEWING FORMS:

These Forms are Adobe PDF Forms and can be completed in Adobe Reader.  They will not work in Word.  If you have trouble viewing these forms, it may be because of your browser. The forms are definitely working if you use MOZILLA FIREFOX. They may be slow to download due to the high resolution….BE PATIENT!!!!

Form B: Release of Liability & Emergency Info. (Return 1 Copy)

Form C: Required Conduct, Prizes & Awards (Return 1 Copy)

Form D: Contestant Profile (Return 5 Copies, 1 for each judge)

Form E: Program and Media Information (Return 1 Copy)

Form F: Program Ad (Return 1 Copy per ad, only required if selling ads)

Form I: Emcee Information (Return 1 Copy)
